How it works

  1. Create a form in your workspace and leave the form mode on Universal (the default).
  2. Add fields from the Add Block catalog - text, choice, contact, media, and special blocks as needed.
  3. Optionally configure conditional logic, multiple endings, themes, and AI follow-ups.
  4. Publish a version, then share the public link, org subdomain, or embed on your site.
  5. Review submissions in Results and track views, starts, and completes in Analytics.

  • Universal does not score correct answers; use Knowledge quiz for graded assessments.
  • Ending-based personality matching belongs on Match quiz mode, not Universal alone.
  • Some add-ons (Contacts sync, Payment, CSV export) remain plan-gated regardless of form type.
