If you find a Formit form that is phishing, harassing, illegal, or otherwise harmful, tell us.

What to include

  1. The full form URL (public link or embed page).
  2. A short description of the issue (phishing, spam, harassment, illegal content, etc.).
  3. Screenshots if helpful.
  4. Your contact email if you want a follow-up (optional).
